Scheduled Maintenance times are created from 0 to 0

Due to a mismatch in the variables used in the web page and in the code inserting the row, scheduled maintenance times are created as starting at 0 and ending at 0. This change updates the field names used in the form to match the field names used in the back end, resulting in the ability to add a scheduled maintenance.
